Custom Protos® Design Form

I understand the price paid on this form is for design production only, and that the amount paid does not contribute to the production of the physical product, should I choose to order it. I understand that the cost of design production is non-refundable, regardless of whether or not the product is ordered.

I understand that trademark and copyright images and phrases cannot be produced.

I understand that I have the right to request changes to my design up to 5 times, and that my design can be applied to as many custom Protos® as I choose to order.
